To request meeting space at the church, please fill in the following form and click "Submit" at the end of the form.

If you wish, you may print this form and fax it to the office Manager at 267-4924, or phone in your request to 267-4946. Or you may simply drop the completed form off at the church office, or place it in the Office Manager mail box in the mail room.

NOTE: If more than one room is checked, and you do not require ALL of the rooms for your event, specify the number of rooms you will need AND your order of preference in the space below.

If you have any notes, special requests, or setup instructions which will help the church office to reserve space for your event, please enter this information in the space below.

For example, please indicate how much time prior to the scheduled start of your event will you require for set-up, and  describe any special equipment (chairs, tables, easels, etc.) you may require.
